  • Professional consultation for parents
  • Screening in the areas of communication, motor development and preschool readiness skills
  • Assistance in placing children with special needs in appropriate exception student education programs

The Capital School District is utilizing the Brookes online system for completion of questionnaires to facilitate the screening process for families, doctors, and community care facilities that may have concerns about the skill development of young children.

In order to submit the Ages & Stages Questionnaires for district review, you will need to go to the following link:

After clicking on this link, the website for Ages & Stages Questionnaires will open.
Then, you should select option – I am completing both the ASQ-3 and ASQ:SE-2 questionnaires.

Please Note: The screening process does NOT continue until the questionnaires are completed. There is NO COST to you.

The results will automatically be returned to Capital School District after you click on the SUBMIT button at the end of the questionnaire. After review by the Early Childhood Team, you will be contacted to discuss next steps. Feel free to call should you have any questions regarding the process.
