Please Note: Any student requesting childcare/alternate transportation for the 2025-2026 school year must complete a new CSD Childcare/Alternate Transportation Form.  A new form must be filled out for each school year. Alternate transportation will NOT be provided if a new form is not received. Forms received after August 11th will not be processed until the second week of September.
