Developmental Screening Questionnaires

Capital School District uses the Brookes online system to help complete developmental screening questionnaires. This process supports families, doctors, and community care facilities that may have concerns about the development of young children.

To submit the Ages & Stages Questionnaires for district review, please visit:

https://www.asqonline.com/family/15f3cc

After opening the link, the Ages & Stages Questionnaire website will appear. Please select:

I am completing both the ASQ-3 and ASQ questionnaires.

Important Information

The screening process cannot continue until the questionnaires are completed.

There is no cost to complete the questionnaires.

After you click the Submit button at the end of the questionnaire, the results will automatically be sent to Capital School District. Once the Early Childhood Team reviews the results, someone from the district will contact you to discuss next steps.

Please contact us at 302-857-4310 if you have any questions about the process.

Thank you for your interest in Capital School District for your early childhood needs.
